She was predeceased by: her father Kurt Kasules; her stepfather Bobby Hayes; and her father-in-law Donald Polacek. She is survived by: her friend Dawn Elaine Polacek of Farmington; her mother Deborah Hayes (Kollwelter) of Wappapello, MO; her husband Dan Polacek; her children, Bryanne Morsch (Tony) and Braden Polacek (Paige); her granddaughters, Vivian Morsch and Cambrie Dawn Polacek; her stepbrothers, Matthew Hayes and Steve Hayes; and her mother-in-law Ellie Polacek of Farmington.
