He is survived by: his father Frank Lawrence Sivak of Charlotte; his wife Joanne Sivak of Denver, Colorado; his children, Jacob Sivak of Denver, Colorado, Marc Sivak of Denver, Colorado and Jessica Phillips of Phoenix, Arizona; his grandchildren, Miles Phillips, John Sivak, Marissa Phillips, Clara Sivak, Gabrielle Sivak and Alexandra Sivak; and his siblings, Scott Sivak of Boston, Gary Sivak (Anne Marie) of Charlotte, Lynda Levine (Howard) of Charlotte and Kim Bishopp (Jim) of Charlotte. He is also survived by his children and grandchildren.
