He was predeceased by: his parents, John Forrest and Martha Forrest (Terrell); and his sister Ella Jane Moore (Daniel Ray Forrest). He is survived by: his wife Janice Earhart of Dover, TN; his children, Melissa (Brian Riddle) of Salem, IL and John Daniel (Angie); and his grandchildren, Jennifer Frauenfelder, Baylee Riddle, Austin Birks, Chase Riddle, Dustin Riddle, Bradley Hodge and Derek Hodge. He is also survived by 6 great grandchildren.
