He is survived by: his children, Kenneth Cook (Mary Jo) of West Plains and Rachel Cook of Dittmer; and his grandchildren, Sean Lutz, Kaitlin Lutz, Blakely Cook and Bethany Bailey. He is also survived by four great-grandchildren.
