He was predeceased by: his parents, Walter Lee Forkum and Ina Bell Earnest Forkum; his son Carrol Dean Forkum; and his siblings, Dayton Forkum, E. J. Forkum, John Forkum, Bill Forkum, Sarah Gaines, Virginia Forkum, Mamie Marie Forkum and Dolly Forkum.
He is survived by: his wife Ramona Jean Lowery; his children, Michael Forkum (Rhonda) of Dexter, Carol Roberts (Guy) of Dexter and Niki Evans (Jack Meese) of Bernie; his siblings, Elvin Forkum of Dexter and Viola Lay of Dexter; his grandchildren, Amanda Forkum, Jay Roberts, Derek Roberts and Cassi Dudley (Ryan); and his great grandchildren, Nolan Krieg, Eland Tittsworth, Jacob Roberts and Makenna Roberts.
