He was predeceased by: his parents, Clyde Cook and Eva Wade Cook; and his brothers, Harold, Clois and Wayne Cook.
He is survived by: his wife Phyllis Blue; his children, Ronald Cook (Glenda) of Niangua, Missouri and Marietta Kimrey (Jerry) of Lebanon, Missouri; his stepchildren, Darris Blue (Jodi) of Albany and Jennifer Perkins (Kyle) of Competition, Missouri; his siblings, Leland Cook of Bolivar, James Cook (Darlene) of Lebanon, Missouri and Ruth Randolph (James) of Lebanon, Missouri; his grandchildren, Adam Kimrey (Jana), Ashley Pospisil (Nathan), Brandon Cook (Hillary), Josh Cook (Amanda), Jessica Decker (John) and Jennifer Ijames; his stepgrandchildren, Danielle Perkins, Ethan Perkins (Elizabeth), Paisley Blue, Dylan Blue and Hailey Blue; his siblings-in-law, Beverly Jean (Gary Smith), Ken of Ozark, Missouri, Lena Rose of Ozark, Missouri and Ronnie Rose of Ozark, Missouri; and his close friend Mary Ann Gravens of Lebanon, Missouri. He is also survived by several nieces and nephews, other relatives, neighbors, church family and dear friends.
In lieu of flowers, memorials have been suggested in memory of Leonard to Lynchburg Cemetery.
