He is survived by: his mother Shirley Ballard (Daniels) of Rogers, Arkansas; his sons, Jared Ballard (Melissa) of Cassville and Justin Ballard (Danny) of Cassville; his sisters, Pam Evans (Les Summey) of Rogers and Alisha Lloyd (Brian) of Angler; his grandchildren, Colt and Nate Ballard; and his friend Joann Johnson.
