He was predeceased by: his parent Glen Smith; and his wife Cindy Addison. He is survived by: his mother Lola Smith (Cuzick) of Cassville, Missouri; his children, Tyler Smith of Cassville, Shane Smith (Jessica) and Sommer Smith of Broken Arrow, Oklahoma; his siblings, Vaughn Smith (Dixie) and Debbie Dorman of Fayetteville; and his grandchildren, Flynn Burnette, Cadyn Smith, Cash Smith, Ryder Smith and Emily Smith.
