He is survived by: his parents, Charles Hazen and Kimberley Montee; his wife Nicole Hazen; his children, Alyssa Maxon, Ian Maxon, Mya Barkley and Carlee Kain; and his siblings, Christa Lawson, Chase Hazen and Jordan Kelley. He is also survived by nieces, nephews, great-nieces, great-nephews, and many extended family.
