She was predeceased by: her parents, Frank Sr. Kalina and Evelyn Kalina (Parkerson); her husband Milton Burl Cook Jr.; and her siblings, Frank Jr., Bill and Donna Lee Puckett.
She is survived by: her children, Milton Cook III (Carrie) of Winston, MO, Michael Cook (Todd) of Greensboro, NC, Mindy Loyd (Michael) of Cameron, MO and Marina Haveman (Dan) of Nogales, AZ; her grandchildren, Milton IV, Kyle, Evan, Brandon, Ashlee, Rachal, Faith and Sydney; her great grandchildren, Kyla, Avasa, Blakely, Beau, Waylon, Grayson and Stassi; and her stepgrandchildren, Hunter McCampbell, Miley McCampbell and Mea McCampbell.
In lieu of flowers, memorials may be made to the American Cancer Society or Graceland Cemetery Association, Weston, MO.
