He was predeceased by: his mother Audrey Hooks; and his stepfather Roy Hooks. He is survived by: his wife Tammy O'Hara; his children, Jennifer Johnson, Steven O'Hara and Christopher O'Hara; his son-in-law Wesley Johnson; and his grandsons, Wyatt and Leo.

In lieu of flowers, the family kindly requests donations be made to the St. Louis Samoyed Rescue or to the Susan G. Komen Breast Cancer Foundation in Steve's memory.
