He was predeceased by: his parents, Sam Cook and Louise Cook; his brothers, Sammy and Harvey Lee Cook; and his parents-in-law, Jim and Roann Wimberly.

He is survived by: his wife Gina (Wimberly); his daughters, Christa (Chuck Davis) and Jessica (Nathan Stamburski); his grandchildren, Austin Davis (Lexie Dill), Preston and Grace Davis; his sister Mary; his brother-in-law Ben Callahan; his brother Mike; and his sister-in-law Glenda Cook.

In lieu of flowers memorials may be made to the Tunnels to Towers Foundation.
