He is survived by: his mother Frances Lee Hulsey (Parks); his wife Susan Elaine Hulsey (Waldo); his children, Bazzel Bates (Anna), Greg Bates (Bonnita), Kelly Miller and Jessica Boyer (Matthew); his grandchildren, Joey, Riley, Lily, Brynn, Ty and Harper; and his siblings, Roxanne Hulsey, Rodney Hulsey, Jimmy Hulsey and Tracy Hulsey.
