He was predeceased by: his grandparents, Joe Cook, Pauline Cook, Earl Offield and Clara Offield; his father Bobby Cook; his son Brian Paul Cook; and his sister Susan Cook.
He is survived by: his mother Jessie Cook; his sons, Nathan Cook (Valerie) of Coos Bay, OR and Quinten Cook; his grandchildren, Justice Cook, William Cook, Madison Cook, Kai Cook, Benjamin Cook, Harley Cook and Zane Cook; and his siblings, Robert Lee Cook (Deborah) of Galena, MO, Tracy Joe Cook and Kym Elaine Jesse of Phoenix, AZ. He is also survived by many nieces, nephews, cousins, and friends.
