Bryan Milburn (Tammy), Lisa Wortham and Heather Hedrick; his grandchildren, Megan, Whitney, Ethan, Stephanie, Tyler, Olivia, Brittany, Jordan and Landon; his great grandchildren, Chloe, Zoey, Jaylynn, Jaxon, Millie, Uriah and Raelynn; and his son John Revelle.
Memorials may be directed in honor of Calvin's memory to Elvins Baptist Church or Heart and Soul Hospice.
