He is survived by: his parents, Donald Cook and Dorothy Goodrich Cook; his wife Shirley of Benton, Missouri; and his sons, Donald J. Cook, Jr. (Tracey) of Edwardsville, Robert Cook of O'Fallon and Lonnie Cook (Mary) of Hillsboro.
