He was predeceased by: his parents, Dewey Cook and Audra Cook (Kastning); and his siblings, Cathy Plaster and James. He is survived by: his sisters, Faye (E. J. Hampton), Carol (Larry Pendergrass), Geneva Davenport and Judy (Ron Hladun). He is also survived by nieces and nephews and many friends.
