She was predeceased by: her parents, Lowell Sapp and Leona Sapp (Lee); and her husband Scott Dean Forbis. She is survived by: her children, Travis Nichols and Tracy Day; her sister Kathy Blackburn (Greg); her siblings-in-law, Steven Forbis (Diane) and Susan Levacy; and her grandsons, Lucas Day and Jonathan Michael Day. She is also survived by many nieces, nephews.
Memorial donations may be made in her honor to American Cancer Society.
