He was predeceased by: his parents, Robert Thomas and Francis Thomas (Taylor); and his children, David, Jr. and Peggy Boaz. He is survived by: his wife Elizabeth "Ann" Dismukes; his son Gary; and his grandsons, Steven, Adam and Rhett Boaz. He is also survived by two brothers and two sisters.
