He was predeceased by: his parents, James Lowell and Vernalee Kathryn Hawkins (Dodson); his granddaughters, Hensley and Haisley Self; and his brother-in-law Cliff Creach.
He is survived by: his daughter Deana Gayle (Tim); his grandson Bryant Dean; his daughter Shannon Burkes (Carl); his grandchildren, Bryant Burkes, Hunter Self (Hailey) and Mckenzi Self of Adrian, MO.; and his siblings, Sue Creach and Jimmy Hawkins of Appleton City. He is also survived by cousins, great nieces and nephews and many great friends.
Memorial contributions may be made to the Appleton City Cemetery.
