He is survived by: his parents, David Leverett and Kelly McCormick Leverett of Annapolis, MO; his siblings, Davey, Jacob Leverett and Kyrsten Leverett; his grandparents, Don, Shirley Parker, Donald, Brenda Leverett and Rick Woodworth (late Kimberly Woodworth); his aunts and uncles, Shelly, Donnie, Larry, Kelly, Jeny, Sherry and Terry; and his cousin Amberlyn McCormick.
