He was predeceased by: his parents, Loyd Cook and Linda Riley Cook; his wife Mary Ann Cook; and his sister Jean Elliott. He is survived by: his children, Lynda Smith (Randy) of Anderson, Sandy Schemahorn (Allen) of Siloam Springs, David Cook (Kim) of Union and Darren Cook (Missy) of Joplin, Missouri; his sister Carolyn Waller of Wichita; his nephew Leland Elliott (Marie) of Sioux City, Iowa; his grandchildren, Tyler Smith (CeCe) of Rogers, Arkansas, Kelsey Smith Bergen (Bo) of Rogers, Arkansas, Caleb Tinker (Jennifer) of Rogers, Joshua, Jared Cook of Union, Brett and Brock Cook of Joplin, Missouri; and his great great grandchildren, Stella, Cora Smith, Evelyn Cook, Blakely Bergen and Madelyn Tinker.

In lieu of flowers, donations can be given to McDonald County School Foundation in care of the funeral home.
