He was predeceased by: his parents, John Hook and Mabel Vander Hook; and his sister Jean Nichols. He is survived by: his wife Sue Mayfield; his children, Joshua David Vander Hook (Christine Mages), Jared Daniel Vander Hook (Alex Elizabeth), Jalene Danielle Vander Hook and Mykaela Joy Denise Townsend (Dakota Townsend); his grandchildren, Conner Vander Hook, Adrian, Raiden Townsend, Ada and Noa Vander Hook; and his brothers, Warren Vander Hook (Kathy Fitzgerald) and Brian Vander Hook.
