He was predeceased by: his grandson Paul Polaczyk; his parents, Edward Polaczyk and Elizabeth Polaczyk; his parents-in-law, Guido and Bernice Dick; his sister Gayla Dick; and his siblings-in-law, Leon Dick, Francis Dick, Jeanette Dick and Barbara Polaczyk.
He is survived by: his wife Mary Rose; his children, Joseph Polaczyk (Jeanne), Anne Marie Igo (Joseph) and Caroline Langfeld (Joseph); his brother Bruce Polaczyk; and his siblings-in-law, Donald Dick, Ruthann Dick and Carol Dick. He is also survived by 6 grandchildren and 2 great-grandchildren.
