He was predeceased by: his parent Elaine Flak (Radomski); and his grandmother Ethel Radomski. He is survived by: his daughter Ashley Stolpa; his grandchildren, Jaylon, Decorreon, Lakoriah and Emari; his brothers, Steve Flak (June) and Wayne Flak; his niece Aimee Hofschulte (Logan); and his nephews, Ryan Flak and Jonathan Flak.
