He was predeceased by: his mother Sarah Dzwonkowski (Henthorne); and his wife Connie Dzwonkowski. He is survived by: his father Daniel Dzwonkowski; his children, Sierra Kramer (Chris Hurlburt), Lyndsey Dzwonkowski (Dakota Olson), Daniel Dzwonkowski (Olivia) and Alyssa Evenson (Alex); his grandchildren, Teagan Bolduan, Ryker Peterson, Sam, Max and Gabriella Evenson; his siblings, Donna Dzwonkowski, Leslie Watson (Bruce) and Greg Dzwonkowski (Tammy); and his nieces and nephews, Haley, Ashley, Jessica, Kaytee, Dustin, Nathan and Jake.
