He is survived by: his mother Dorothy Flicek (Kukacka); his wife Carolyn; his sister Debra Pelowski (Michael); his daughters, Chris Shaw (Jason) and Sheri Miller (Paul); his grandchildren, Mackenna Zelenka, Trevor Shaw, Victoria Shaw, Karissa Shaver (Justin Creel), Walker Miller, Andrew Miller and Faith Miller; his great grandchildren, Carter and Jeremiah Creel; and also, Ellen Flicek, Julia Busch, Leo and Mary Kukacka. He is also survived by numerous nieces and nephews.
