He was predeceased by: his parents, Kenny Formo and Leone Formo; and his brother Craig Formo. He is survived by: his wife Patty; his children, Ben Formo (Krista Larum) and Angie Cordner (Cameron); his grandchildren, Brandon Barrett, Caleb Formo, Ava Formo, Ivan Cordner and Dax Cordner; and his brother Tim Formo.
