He is survived by: his children, Jake and Jenna Flavin (Mike); the mother of his children Cindy Madison; his grandchildren, Josh Madison, Parker, Rylan Rogers, Skylar and Scarlet Flavin; and his brothers, John (Linda) and Jeff Madison (Colleen). He is also survived by nieces, nephews, other family, and friends.
