He was predeceased by: his parents, Ralph Otis and Mae Otis (Johnson); his daughters, Deborah and Karen; and his granddaughter Alyssa.

He is survived by: his wife Marjorie Adams; his children, David Lee Otis of Mabel, MN, Paul Otis (Tammy) of Pine City, MN and Annette Larsen (Paul) of Brainerd, MN; his grandchildren, Mindy, John (Ashley), James (Jenae), Joshua, Jessica (Ashok), Christina and Nathan; and his great grandchildren, A. J. and Louisa.
