He was predeceased by: his parents, Wilbert Cook and Geraldine Cook (McBride); his brother Jimmy; and his grandparents, James, Mazie McBride, Wilbert Sr. and Dorothy Cook.

He is survived by: his wife Bonni; his children, Nikki Elton (Kenny) and Nate Cook (Korine); his grandchildren, Dylan, Cole and Gracelyn; his siblings, Pat Cook (Martha) and Kathie Moberg (Pete); his sister-in-law Terry Placek; his pet Winchester; his cousin Bob; and his granddaughter Gracie. He is also survived by nieces, nephews, cousins, friends.

In lieu of flowers, memorials are preferred to the family that will be donated to Mike's favorite charities.
