He was predeceased by: his mother Deborah Jo Cook (Dykstra); and his grandfather Leroy Dykstra. He is survived by: his wife Katie Stevens of Luverne, Minnesota; his children, Monalisa Eileen Cook of Volga, Kisa Ann Stevens of Pipestone, Minnesota, Remy Zachary Cook of Pipestone, Minnesota, Aeriel Bernice Cook of Pipestone, Minnesota, Leroy Allen Cook of Pipestone, Minnesota and Ayla Lynn Cook of Pipestone, Minnesota; his father Charles Jackson Cook, Jr. of Brookings; his siblings, Trey Michael Cook (Jennifer) of Brookings, Moiria Elizabeth Curry (Nick) of Brookings, Taylor John Cook of Brookings, Shannon Marie Longworth (Kory) of Jacksonville and Amanda Nicole Cook of Fleming Island, Florida; and his grandmother Bernice Parsons Dykstra of Pipestone, Minnesota. He is also survived by many loving nieces, nephews, aunts, uncles, cousins, and friends.
