She was predeceased by: her parents, Percy Forrest and Della Forrest (Leslie); and her siblings, Art Forrest and Bonnie. She is survived by: her spouse Leslie "Les" Mullinix; her children, Pam Jackson (Dan), Brian Stephan and Shawn Shelstad (Lori); her grandchildren, Thomas Jackson (Lindsey), Jace Jackson (Kayla), Paige Callahan (Dan), Haily Murry (Nick) and Hunter Shelstad (Daisy); and her great grandchildren, Addison, Iris Jackson, Beau, Nora Callahan, Maverick Murry and Bennett Jackson.

In lieu of flowers, memorials are preferred.
