She is survived by: her husband Ted Slezak; and her siblings, Thomas Charles Crosby III, Lesa Anne Chambers (Dave), Sean Kelly Crosby and Elizabeth Jo Fox (Todd Cryderman).
Actively supported many associations such as Cancer Societies, American Heart and Lung Association, and Mothers Against Drunk Driving.
