He was predeceased by: his parents, Sidney Smith and Elva Smith Myhre (Brue); and his sister June Findlay. He is survived by: his friend Sandi; his wife Sandi; his children, Shelli Smith of Denver, CO and Stuart Smith; his daughter-in-law Susan; his grandchildren, Olivia and Lauren of Black Forest, CO; and his siblings, Allie Enga (Luther Eidem) of Cottonwood, MN, Alvin Smith (Marilyn) of Minneota, MN and Sylvia Swanson (Wayne) of Breezy Point, MN. He is also survived by many nieces, nephews, cousins, and friends.

Any gifts received on Sam's behalf will be donated to the Southwest Sportsmen's Club to maintain local wildlife areas.
