He was predeceased by: his mother Sandie. He is survived by: his father Tom Sr.; his stepmother Jan; his siblings, Tom Jr. (Kelly Krebs), Teresa Schlegel (Joe), Tracy Mindiola (John) and Kristine Clemens; his nieces and nephews, Jacob Schlegel (Carly), Tyler Schlegel (Rachel Hogan), Christian Mindiola, Kaylah Fryklund (Pat) and Julianna Mindiola; his great-niece Reagan Schlegel; his aunts and uncles, Bonnie (Paul Clark), Alyce Lee, LuRetta Haire and Mike (Mary Lou Campion); his former wife Sophie; and also, Ava and Joshua.
Due to COVID regulations, space is limited to 50 attendees.
