He was predeceased by: his father James Hazzard. He is survived by: his mother Grace Hazzard; his wife Lisa Hazzard; his daughter Serena; his brothers, Phil Hazzard and John; his sister-in-law Jennifer Hazzard; his niece Claudia; his nephew Henry; his sister-in-law Jill McDade; his niece Mazie (Justin Gengler); his great-nephew Adrian; his nephew John McDade; and his great-niece Phoebe. He is also survived by cousins, family, lifelong friends, goddaughters, and coworkers.
