He was predeceased by: his mother Lydia Chalupsky (Mikyska); his daughter Paige Chalupsky; his siblings, Louise Chalupsky and Louis "Butch" Chalupsky Jr.; and his brother-in-law Robert Kozisek.
He is survived by: his father Louis Chalupsky; his wife Laura Chalupsky of Beroun; his daughter Nichol Herzog (Michael) of Auburn, Washington; his grandson Dylan Herzog of New Haven, Connecticut; his sister Lorraine "Toots" Kozisek of Pine City; and his siblings-in-law, Karen Chalupsky of Orange, California and Denny Grover of Finlayson. He is also survived by many nieces and nephews; relatives; and friends.
