He was predeceased by: his parents, Curtis Ellsworth and Bernice Holtan (Slinde); and his sisters, Patricia and Mary Beth. He is survived by: his wife Barbara; his children, Todd (Jody), Christina (Kevin) and Tim (Amy); his stepson Brian; his grandchildren, William, Brittany, Alex (Olivia), Chloe Delwiche (Tyler), Nick (Nicole), McCall and Mitchell; his great grandchildren, Israel, Emelio, Mackenzie, Jackson and Ariana; his siblings, Sandra Stein (Bob), Linda Hein and Richard (Peg); and his sister-in-law Marlene Nelson. He is also survived by nieces, nephews, cousins and many dear friends.
