We are sad to announce that on November 22, 2021, at the age of 94, Hazel Lorraine Chase (Minneapolis, Minnesota) passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She was predeceased by: her parents, William Frolichman and Sarah Frolichman; her husband Jack Chase; and her siblings, Peggy Kane, Marvin and Harold Frolichman.

She is survived by: her daughters, Rae Berman (Carolyn DeLuca) and Dede Smith (David); her grandchildren, Sarah Berman-Young, Lisa Berman (Joseph Tran), Noah Berman (Rocco Landi), Jaclyn Berdugo (Philippe) and Ben Smith; her great grandchildren, Julian, Leta Young, James, John and Juliet Berdugo; and also Carol Polk. She is also survived by many loving nieces and nephews.
