He was predeceased by: his aunts, Bonnie Monley and Pam Raymond; and his uncle Carl Bates. He is survived by: his friend Whitney Berg; his parents, Lauralee Hlavinka (Guy) and Larry (Terri); his siblings, Matthew (Caitlin), Timothy (Megan), Joseph Hanson (Nicci), Katie (Ross), Alexander Hlavinka, Rachel Palmateer (Ben) and Thomas; and his uncles, Bryan (Linda), Rob Raymond, Bill Bates and Bud Bates (Becky). He is also survived by his nieces, nephews, and many relatives and friends.
Visitation was held on Saturday, February 15th 2025 from 9:00 AM to 11:00 AM at the Roberts Family Funeral Home (555 SW Centennial Dr, Forest Lake, MN 55025). A memorial service was held on Saturday, February 15th 2025 at 11:00 AM at the same location. A funeral service was held on Saturday, February 15th 2025 at 12:00 PM at the same location.
In lieu of flowers, if you would like to make a monetary donation in Sam's honor, the family asks that contributions be given to the family directly in support of the ceremony and/or to a local recovery centered program of your choosing.
