He was predeceased by: his parents, Corky Ellingson (Nancy Klipstein) and Georgia Ellingson (Adrian); his sons, Darin Slack and Tim Slack; his brother George Slack; his nieces, Andrea Slack and Becky Slack Peterson; and his nephew Taylor Bergsrud.

He is survived by: his wife Mona Slack; his children, Tom Jr. Slack (Charlotte) of Evansville, MN, Debbie Otto (Rob) of Evansville, MN and Marc Slack (Heather) of Evansville, MN; his siblings, Larry Slack (Linda) of Brandon, MN, Greg Slack (Sandy) of Evansville, MN, Sonya Byrne (Bill) of Welch, MN and Roger Slack (Rita) of Boise, ID; and his sister-in-law Sharon Slack of Alexandria.
