He was predeceased by: his parents, Morris Kolk and Henrietta Vander Kolk (Vanden Bosch); his sister Hazel; his siblings-in-law, Daryl, Larry Tolsma and Mary Tolsma; his daughter Jana; and also, Pete and Nellie Tolsma.
He is survived by: his wife Donna (Tolsma); and his daughters, Evon and Sandy.
