He was predeceased by: his father Carl "Dick"; and his brother Rick. He is survived by: his wife Karen; his daughters, Sarah Kulavik (Adam) and Shannon Van Alstine (Nicholas); his mother Rosemary; his siblings, Steve (Jodi), Kathy Shanahan (Marty) and Kari (Dave Altendorf); and his sister-in-law Lynne Hess (Greg). He is also survived by many in-laws, cousins, nieces, nephews and friends.
