He was predeceased by: his parents, Samuel Mack and Beverly Blackburn; his stepfather Rex Blackburn; and his grandparents, Alice and Harold Rutherford.

He is survived by: his wife Marcy (Anderson); his children, Derek Blackburn (Cayla), Justin Blackburn (Brie) and Jamie Leidman (Tyler); his grandchildren, Maxwell, Zoey and Noah; and his sister Sherry Safratowich (Joe).
