He was predeceased by: his parents, Fredrick Cook and Bernice Cook; his siblings, Bob, Nancy and Michelle; his former wife Theresa; and his sons, Tim and Scott.
He is survived by: his daughter Theresa; his former wife Shirley; his sons, Dale (Lynda) and Mark (Dawn); his grandchildren, Dawson and Rochelle (Justin Lewis); and his great grandchildren, Kendall and Layla.
The family has requested no gifts, but instead take a youngster bowling who might not otherwise get the chance to go bowling, in honor of Don.
