She was predeceased by: her former husband Leo Carpenter; her husband Vince Otis; her parents, Joseph Hajek and Florence Hajek; and her brother Harry.
She is survived by: her children, David (Janice), Terry (Barb Schifsky), Diana Saver (Dan) and Jerome (Debbie); her nephew Jeff Hajek; and her close friend Shirley Miller. She is also survived by many grandchildren and great-grandchildren.
