He was predeceased by: his parents, Daniel Hayward and Luella Hayward (Bigelow); his daughter Ann Warren; and his brother David Eastman. He is survived by: his wife Patricia Carey; his children, Kathy Hayward, Teri Archuleta (Jason) and Carey Hayward; his grandchildren, LeAnn Stafford, Tyler Warren, Sophia Archuleta and Samuel Archuleta; his sister Betty Mason; his niece Sue; and his nephews, Patrick and Daniel. He is also survived by many other relatives and friends.
