He was predeceased by: his father Gary Bornhoft. He is survived by: his children, Alex, Jake and Zach; his wife Patty (Krier); his mother Sandra; and his siblings, Amy DeBondt (Jeff), Joel (Lora) and Chad (Beth). He is also survived by numerous nieces, nephews, aunts, uncles, in-laws, cousins and friends.

In lieu of flowers, the family asks that you donate to the Prostate Cancer Foundation and pray that someday we will find a cure for this unforgiving disease.
