He was predeceased by: his wife Helen; and his son Carl Bolks. He is survived by: his daughters, Kathie Eding (Mike) and Diane Disselkoen (Dan); his grandchildren, Kelli Klingenberg (Gary), Marcy Burton (Travis), Laura Bolks, Nicholas Disselkoen (Sarah), Tony Disselkoen (Monica Slagh) and DJ Disselkoen; and his great grandchildren, Ethan, Tyler Klingenberg, Traycen, Maddy, Max Burton, Abigal Disselkoen and Tanner Disselkoen. He is also survived by many nieces and nephews.
In lieu of flowers the family is asking memorial contributions be sent to Hospice of Holland.
